Your Wellness Partnership

At Evolve Health & Wellness, our partnership programs are designed around one principle: meaningful results take time. Whether you're optimizing hormones, transforming your skin, restoring sexual vitality, or reshaping your body composition, the protocols we build for you are progressive — each visit refines your plan based on how your body responds.

That's why every partnership at Evolve is built on a six-month commitment. Not because of fine print — because of physiology. Your body needs time to adapt, respond, and reveal the results you came here for.

Why Six Months?

Metabolic & Hormone Health

Hormonal optimization is not a single adjustment — it's a process. Lab work must be drawn, protocols initiated, levels re-checked, and dosages fine-tuned. Most patients begin to feel meaningful changes within 4–8 weeks, but true metabolic optimization — stable energy, improved body composition, restored vitality — unfolds over months. Rushing this process compromises both safety and results.

Aesthetic Partnerships

Skin remodeling, collagen stimulation, and texture improvement follow your body's natural healing timeline. Treatments like RF microneedling, laser resurfacing, and injectables are staged intentionally — each session builds on the last. Visible transformation typically requires 3–6 sessions spaced weeks apart. A six-month partnership ensures you complete a full treatment arc and enjoy the results.

Sexual Health

Restoring sexual wellness involves careful evaluation, treatment initiation, and ongoing optimization. Protocols like the P-Shot, Empower RF, or oral medication management require follow-up assessments to ensure safety, efficacy, and patient satisfaction. A six-month window allows us to deliver the full scope of care.

Partnership Commitment Period

All Evolve Health & Wellness partnership memberships require a minimum six (6) month commitment beginning on the date of enrollment. This applies to both Aesthetic Partnerships and Health & Wellness Partnerships.

During your commitment period, you'll receive the full benefit of your selected partnership tier, including scheduled treatments, provider consultations, and protocol adjustments as outlined in your membership agreement.

Cancellation & Early Termination

We understand that life circumstances change. If you need to cancel your partnership, we ask for the following:

30-Day Written Notice Required

All cancellation requests must be submitted in writing at least thirty (30) days prior to your next billing date. You may submit your cancellation request by email to info@evolvehealthfl.com or in person at our office. Verbal cancellation requests cannot be accepted.

Early Termination Fee

If you cancel your partnership before completing the six (6) month minimum commitment period, an early termination fee of $199.00 will apply. This fee reflects the discounted pricing, reserved appointment availability, and clinical planning invested in your care from the start of your partnership.

After the Commitment Period

Once you've completed your initial six-month commitment, your partnership will continue on a month-to-month basis. You may cancel at any time with 30 days' written notice and no additional fees.

Billing & Payments

Partnership fees are billed monthly on the date of enrollment via the payment method on file. It is the member's responsibility to maintain a valid payment method. You may update your payment information at any time through your patient booking portal.

If a payment fails due to an expired card, insufficient funds, or other payment issue, a $50.00 reinstatement fee will be applied. We will attempt to contact you and re-process the charge. Accounts with balances past due for more than 15 days may result in suspension of partnership benefits until the balance — including any reinstatement fees — is resolved.

Partnership pricing is locked for the duration of your commitment period. After the initial six months, pricing may be adjusted with 30 days' notice.

Modifications & Transfers

Evolve Health & Wellness reserves the right to modify partnership offerings, treatment protocols, and scheduling with reasonable notice. Partnership memberships are non-transferable and may not be shared with other individuals.

Unused treatments or sessions within a billing cycle do not roll over to subsequent months unless otherwise specified in your specific partnership tier agreement.

Our Commitment to You

We entered this partnership with you because we believe in the plan we've built together. Our providers are personally invested in your progress. The six-month commitment exists so we can do our best work — adjust your protocols, track your progress, and deliver the results that brought you through our door.
