Payment Policy
Evolve Health & Wellness is a concierge wellness clinic. Payment is due at the time services are rendered unless other arrangements have been made in advance.
Accepted Forms of Payment
We accept the following forms of payment:
- Cash
- Credit and debit cards (Visa, Mastercard, American Express, Discover)
- HSA and FSA cards (for eligible services)
- Cherry financing (subject to approval)
Financing Options
We partner with Cherry to offer flexible payment plans for qualifying patients. Cherry allows you to split the cost of treatments into manageable monthly payments. Apply in seconds with no impact to your credit score.

Insurance
Evolve Health & Wellness is a cash-pay clinic. We do not bill insurance directly. However, some services — particularly hormone therapy and certain lab panels — may be eligible for reimbursement through your insurance provider.
We are happy to provide you with a superbill or itemized receipt that you can submit to your insurance company for potential out-of-network reimbursement. Please check with your insurance provider for details on your specific plan coverage.
Pricing Transparency
We believe in transparent pricing. Treatment costs are discussed during your consultation before any services are performed. You will never be surprised by charges you did not agree to.
Pricing for specific services may be listed on individual service pages. All quoted prices are subject to change and may vary based on individual treatment needs.
Outstanding Balances
Any outstanding balances are due within 30 days of the date of service. Accounts with balances past 30 days may be subject to restrictions on future scheduling until the balance is resolved.
Refund Policy
Due to the nature of medical and aesthetic services, refunds are generally not available once a treatment has been administered. Refunds for unused prepaid packages or memberships are handled on a case-by-case basis. Please speak with our team if you have concerns.
